Indications for ICSI: ICSI is typically recommended for individuals or couples facing specific fertility challenges, including:

  • Severe Male Factor Infertility: Conditions such as low sperm count (oligospermia), poor sperm motility, or abnormal sperm morphology.

  • Previous Failed IVF Cycles: Cases where prior IVF attempts resulted in fertilization failure.

  • Sperm Retrieval Issues: Situations where sperm must be obtained surgically, such as in cases of obstructive azoospermia or testicular sperm extraction (TESE).

  • Unexplained Infertility: When the cause of infertility remains unidentified despite thorough evaluation.

  • Frozen Sperm Use: When using frozen sperm, especially when the motility of the thawed sperm is significantly reduced.

Pre-ICSI Assessment and Counseling: Before the ICSI procedure, a comprehensive evaluation is conducted, including medical history, physical examinations, hormonal assessments, and semen analysis. Counseling is essential to inform individuals or couples about the ICSI process, discuss expectations, and address any emotional concerns. This preparation helps ensure that patients make informed decisions regarding their treatment options.

Ovarian Stimulation and Egg Retrieval: Similar to conventional IVF, ICSI begins with ovarian stimulation, where fertility medications are administered to promote the development of multiple eggs. Monitoring through blood tests and ultrasounds is conducted to assess ovarian response. Once the eggs reach maturity, a minor surgical procedure called transvaginal ultrasound aspiration is performed to retrieve the eggs from the ovaries.

Sperm Preparation and Selection: On the same day as egg retrieval, a sperm sample is collected from the male partner or sperm donor. The sperm is processed in the laboratory to select the healthiest and most motile sperm. This preparation is critical to ensure that the best sperm are chosen for injection during the ICSI procedure.

Micro-Manipulation Technique: The ICSI procedure involves using a specialized microscope and micromanipulation tools. A single sperm is selected and injected directly into the cytoplasm of a mature egg using a fine glass needle. This technique bypasses many barriers that sperm may encounter during natural fertilization, increasing the chances of successful fertilization.

Fertilization and Embryo Culture: After the injection, the eggs are monitored for signs of fertilization, typically assessed after 16-18 hours. Fertilized eggs (embryos) are then cultured for several days to observe their development. The embryologist selects the healthiest embryos for transfer into the woman’s uterus.

Embryo Transfer Procedure: Similar to IVF, the embryo transfer in ICSI is typically performed 3 to 5 days after fertilization. A thin catheter is used to place the selected embryos into the woman’s uterus. This procedure is generally painless and may involve transferring one or more embryos, depending on the patient’s age, medical history, and preferences.

Risks and Considerations: While ICSI is generally safe, there are potential risks involved, including complications related to the egg retrieval procedure, such as infection or bleeding. There is also a risk of multiple pregnancies if more than one embryo is transferred. Healthcare providers discuss these risks with patients to ensure they are fully informed.
